Primary care is the backbone of a high-performing health care system. Primary care providers are best positioned to manage patients’ overall health through first-contact access, comprehensive management of chronic conditions, longitudinal relationships, and care coordination. As policy makers and payers continue to test value-based programs to improve outcomes and contain costs, understanding the role of value-based care enablement in primary care is important.
