Individuals with heart failure (HF) are at increased risk for avoidable hospitalization after discharge. We used health insurance claims to study this. We found that, following an HF admission, delayed filling of a newly prescribed HF medication increased the likelihood of an avoidable readmission compared with patients who filled their prescription within 30 days. Late filling also was associated with increased 6-month total and medical costs.
